Registration forms are arriving faster than they can be logged into our database, so that requests for acknowledgements and receipts may be delayed. A list of registrants will be posted at this website and updated periodically, this list may provide you with the earliest confirmation that your registration has been processed.

All attendees, including invited speakers, must register. All registered participants will receive a conference bag and materials, including a conference badge securing admission to the Opening Ceremony, Welcome Reception, the Tuesday Evening Gala, and the exhibition. Conference badges for scientific registrants will also secure admission to the scientific sessions. Admission to these various events will be by badge only. General registration will begin on Thursday, August 8, on the fourth floor of the Washington State Convention and Trade Center (not in the Sheraton Seattle Hotel) from 10:00 a.m. until 7:00 p.m. It will continue daily in the convention center. Congress registrants with special needs arising from disabilities are invited to contact the Organizing Chairman or the Congress Secretariat to discuss appropriate accommodations.

Early Registration / Confirmation of Registration

*Before June 1 fee listed first. After June 1, use higher fee.

*Registration forms must be postmarked on or before June 1, 1996, to be eligible for reduced registration fee. On site registration will be accepted at the higher rate. Registrations received via fax must include credit card payment. If you wish to receive an e-mail acknowledgment of receipt of the registration form and payment, please indicate this on the registration form in the appropriate box.

Cancellation Policy

Cancellations and requests for refunds should be made in writing to the Congress Secretariat. For cancellations received before June 1, 20% of the total remittance will be deducted; after June 1 and before July 8, 50% of the total remittance will be deducted. Fees will not be refundable after July 8, 1996.

Payment

Payment of fees must accompany all registration forms. No registration will be confirmed until payment is received. All payments must be in U.S. dollars. Payments may be made with VISA or MasterCard or by interbank transfer. Because of the high cost of processing interbank transfers, registrants paying in this way should add $35 to their total remittance as indicated on the registration form. This fee does not apply to the other forms of payment. Submit interbank transfers to:

Checks, denominated in U.S. dollars and drawn on U.S. banks will be accepted. These should be made payable to CGA 17, Inc., and marked "For Deposit Only". Personal checks drawn on non U.S. banks cannot be accepted. Fees for workshops, excursions and banquet are separate from the registration fee but should be included in the total payment.
